Job Description

Te mahi/ The role

As the Te Puna Ora (TPO) – Programme Coordinator, you will play a key role in coordinating and delivering support programmes for patients and their families within the Te Puna Ora o Tōtara service. Working closely with the Allied Health Manager and multidisciplinary teams (MDT), you'll ensure the smooth operation of programmes that meet the needs of our community.

Key responsibilities include:

  • Leading and coordinating patient and family programmes.
  • Organising in-house and off-site group programmes with affiliated hospice networks.
  • Collaborating with MDT and volunteers to facilitate programmes.
  • Promoting Te Puna Ora programmes to internal and external networks.
  • Managing the waiting list for programme participation.
  • Handling referrals and ensuring clear communication with patients and whanau.
  • Documenting patient participation and evaluating programme effectiveness based on feedback.

We are seeking someone with a strong understanding of the principles of the Treaty of Waitangi, along with experience in programme coordination, preferably within healthcare or community services. You should have excellent organisational and communication skills, be culturally aware, and capable of managing multiple programmes and relationships.

  • Experience working with volunteers and multidisciplinary teams.
  • Strong competence in record-keeping and data management.
  • Cultural awareness and respect for the diverse spiritual beliefs, practices, and lifestyle choices of the communities we serve.
  • Sensitivity and respect in all interpersonal relationships.
