Tōtara Hospice is the leading provider of palliative care in South and Southeast Auckland. And right now, we’re searching for a compassionate Psychosocial Assistant to support our multidisciplinary team to deliver our vision: Making the world a better place for dying people.

Te Mahi | The Role...

You will work under the guidance and delegation of registered Social Workers and Counsellors, supporting the psychosocial team in delivering high-quality, person-centred care. Key responsibilities include:

  • Supporting the implementation of care plans and follow-up actions.
  • Providing practical support to patients and families (e.g. benefits, housing, funeral planning).
  • Support bereavement follow-up calls or group sessions.
  • Assisting with Advance Care Planning documentation and discussions.
  • Helping patients and families navigate health and social systems.
  • Maintaining accurate and timely documentation in patient records.
  • Supporting data collection for audits, service evaluation, and quality improvement.
  • Attending team meetings and contributing to service development initiatives.

Our ideal candidate is someone who works well in a team environment and brings a professional, mature approach to their work—while also knowing how to positiviely influence team collegiality and maintain professionalism.. You’ll have the ability to “read the room,” which is especially important in the sensitive and emotionally complex environment we work in. You’re proactive in getting things done, solutions-focused when faced with challenges, and committed to delivering high-quality support to patients, whānau, and the wider team.

You will also bring:

  • Competence in using clinical information systems (MedTech) and Microsoft Office (Word, Excel, Outlook, PowerPoint).
  • Ability to maintain professional development in digital literacy and adapt to new technologies.
  • Strong communication and organisational skills.
  • Experience in health, social services, or community support roles is advantageous.
  • Ability to work under delegation and collaboratively with the multi-disciplinary team
  • seek guidance and supervision when tasks fall outside scope or competence
  • Full and clean drivers licence
